Job summary

Live Well Homecare seeks a qualified therapist to oversee and deliver Occupational, Physical, and Speech Therapy services for inpatient and outpatient populations, developing and supervising clinical care in line with departmental objectives.

Responsibilities include coordinating therapy services, supervising staff, and ensuring adherence to policies and safety standards, while communicating with teams, patients, and families and supporting program development and quality improvement initiatives.

Qualifications

• Licensed Physical Therapist or Occupational Therapist required.
• Doctorate in Physical Therapy or Master's in Occupational Therapy preferred.
• Minimum 5 years of clinical experience preferred.

Responsibilities

• Assessing patient needs, creating and updating individualized treatment plans, and evaluating progress.
• Coordinating therapy services, supervising clinical staff, and ensuring adherence to policies, safety, and quality standards.
• Communicating with interdisciplinary teams, patients, and families, handling conflict, and promoting collaboration.
• Participating in student training, mentoring staff, and fostering team relationships.
• Maintaining documentation, managing departmental logistics, and supporting program development and quality improvement initiatives.
• Assist in discharge planning and recommend adaptive equipment, ensuring evidence-based practices are utilized.
