Job summary

Cynet Health is seeking a licensed Physical Therapist for a 13-week home health assignment in Fayette County, Uniontown. The clinician will travel to patients’ homes, deliver PT services, and participate in weekly meetings using HomeCare HomeBase EMR.

Candidates must have at least 1 year of adult home health experience, a current PT license, CPR/BLS, reliable transportation, and a valid driver’s license with auto insurance. Mileage may reach 50–75 miles and no guaranteed hours apply.

Description:This is a home health assignment.The clinician will be responsible for driving to patients' homes daily and attending weekly meetings.The clinician must have a vehicle, a valid driver’s license, and auto insurance.The mileage radius for this job is 50-75 miles.This may result in drives that are longer than 20 minutes between patients.The clinician must be comfortable with this arrangement.The clinician must accept every patient on their schedule regardless of how far they are from their previous appointment.Declining patients may result in cancellation.All documentation must be completed within 24 hours of the appointment with the patient.Delayed and incomplete documentation that impacts billing and follow-up patient care may result in cancellation.There are no guaranteed hours with this contract.The clinician must achieve the weekly productivity points of 30, which equates to 40 hours per week.All overtime must be pre-approved.If the clinician is not receiving enough patients each week to meet their financial needs, they will have the opportunity to float to another nearby clinic to pick up additional patients.Experience in home health is required.First-timers will not be accepted.The clinician must be open to traveling up to 75 miles from the assigned care center.Local candidates are accepted at the same rate.This position requires proficiency in HomeCare HomeBase EMR and recent experience.
