Job summary

Cynet Health in Oregon is seeking a Physical Therapist for outpatient services on a 26-week assignment. The role requires a DPT, state licensure, and CPR/BLS; weekend on-call coverage is required. Day shift with 8–10 hour days.

You will assess and treat patients, develop individualized plans, document in the EMR, and collaborate with an interdisciplinary team to restore function and mobility.

Qualifications

• Two years of Physical Therapy experience preferred.
• PT licensure in state of practice required.
• CPR or BLS certification from an approved vendor required.
• Doctor of Physical Therapy (DPT) degree required; only if obtained license on or after January 2016 or foreign equivalent.
• Weekend coverage and on-call responsibilities are required.

Responsibilities

• Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions while meeting department objectives.
• Serves as co-leader in an interdisciplinary approach to patient groups.
• Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
• Uses appropriate clinical techniques and tools to establish individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques.
• Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
• Performs other job-related duties as assigned.
