Job summary

Soleo Health seeks a Full-time Therapy Care Pharmacist in Centennial, CO to support our Specialty Infusion Pharmacy. You will collect data, optimize care plans, and dispense specialty medications while coordinating with the broader pharmacy team.

The role requires Colorado Pharmacist licensure, home infusion or specialty pharmacy experience, and a Doctor of Pharmacy (PharmD) or equivalent. On-site schedule is Mon–Fri, 8:30am–5pm, with on-call rotation.

Qualifications

• Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience.
• Post graduate residency and/or training.
• Active state licensure is required.
• Additional specialty certification or training may be required.
• Prior experience in home infusion and/or specialty pharmacy preferred.
• Excellent clinical and customer service skills.
• Basic to intermediate proficiency with Microsoft Excel, Word and Outlook.

Responsibilities

• Collect clinical data for patients, caregivers, and nurses and assess data to monitor efficacy and safety of prescribed specialty therapies.
• Collaborate with prescribers to implement or modify treatment plans.
• Act as a subject matter expert and clinical knowledge resource for patients, prescribers, and team members.
• Coordinate activities with pharmacy team members to ensure efficient, patient-focused care and optimal outcomes.
• Document all patient interactions and care coordination in the designated system of record.
• Dispense specialty medications and supplies.
• Assist in orientation and training of new pharmacy staff.
• Supervise therapy care specialists and support personnel.
• Ensure compliance with state and federal law, accreditation standards, and company policies.
