Job Description :
- Collaborates with the interdisciplinary team, the patient/family/caregiver, and the patient’s primary care provider to develop a holistic plan of care focused on achieving the patient/family physical therapy goals.
- Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value.
- Promotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent physical therapy in the patient’s home.
- Practices independently in the patient’s home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
- Acts as case manager and team lead for patients requiring home health therapy services, especially when nursing is not involved or physical therapy is the primary need.
- Assesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.
- Provides physical therapy treatment to the patient/family/caregiver as ordered.
- Completes accurate clinical documentation to support services provided within specified timeframes.
- Provides physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth, development, and life stages.
- Provides instruction to the patient/caregiver and revises the treatment plan as appropriate.
- Maintains communication regarding patients with the case manager and other disciplines via care conferences or other methods.
- Reports acute medical problems and condition changes to the primary care provider.
- Provides supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.
- Completes regulatory required assessments and medication reconciliation within specified timeframes.
Qualifications :
- 1-2 years outpatient rehabilitation setting experience.
- At least two (2) years of physical therapy experience highly preferred.
- One (1) year experience in home health or other health setting with geriatric emphasis.
- Current license in Physical Therapy or ability to obtain prior to hire.
- American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire.
- Current Driver’s license.
- Ability to work without direct supervision.
- Provides care appropriate to the age of patients served.
- Knowledge of physical therapy assessment, treatment methods, modalities, and equipment.
- Ability to maintain effective relationships with other healthcare professionals.
- Proficiency in using technology in a home care setting.
- Demonstrates professionalism when interacting with patients, families, physicians, and hospital staff.
