Pro Rehab in Fulda, MN is hiring a Therapist (PT, OT, SLP) for an On-Call role at Maple Lawn Senior Care. New grads are welcome. There are no productivity requirements, and a competitive wage plus benefits are offered.

Responsibilities include assessing and evaluating patients, developing and implementing treatment plans, delivering skilled therapies, educating families and staff, and collaborating with an interdisciplinary team to promote independence and quality of life.

Qualifications

• Graduate from an accredited therapy program and have a Bachelor's Degree.
• Current licensure for MN, ND, or IA.
• Knowledge of state, federal, and local home health regulations.
• Capable of performing essential job functions with or without accommodations.
• Ability to communicate in English, verbally and in writing.

Responsibilities

• Assess & evaluate patients to identify needs based on discipline scope.
• Develop and implement individualized treatment plans and rehab programs.
• Provide skilled therapies and therapeutic activities to address deficits.
• Educate patients, families, and staff on pain management, safety, and daily functioning.

Pro Rehab, an established Rehab company with over 35 years of successful business, is hiring a Therapist (PT, OT, SLP) to join the team at Maple Lawn Senior Care in Fulda, MN. We offer a competitive wage and benefits. No productivity requirements! New grads are welcome to apply. Position type: On-Call. Wage range: PT & OT $60/hour, SLP $55/hour. Location: 400 NE 7th St, Fulda, MN 56131.

Responsibilities

• Assess & Evaluate Patients: Conduct comprehensive evaluations to identify physical, cognitive, communication, and/or swallowing needs based on discipline-specific scope of practice.
• Develop & Implement Treatment Plans: Create and carry out individualized treatment plans and rehabilitation programs to address functional deficits and promote independence.
• Provide Therapeutic Interventions: Deliver skilled, hands‑on therapy and therapeutic activities, including exercises, manual techniques, adaptive strategies, assistive device training, and communication or swallowing interventions as appropriate.
• Educate & Support Patients, Families, & Staff: Provide education such as pain management, fall prevention, home safety, coping strategies, communication techniques, and strategies to improve daily functioning and quality of life.
• Monitor Progress & Document Care: Track patient outcomes, document evaluations and progress, and modify treatment plans based on patient response and clinical judgment.
• Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, therapists, educators, and other healthcare professionals to ensure coordinated, comprehensive patient care.

Qualifications
