Pro Rehab is hiring a Therapist (PT, OT, SLP) for Maple Lawn Senior Care in Fulda, MN. This on-call role offers competitive pay with no productivity requirements, welcoming new grads.

Wage ranges are $60/hour for PT/OT and $55/hour for SLP. Location is 400 NE 7th St, Fulda, MN 56131. On-call schedule and benefits apply.

Qualifications

• Must be a graduate from an Accredited Therapy Program.
• Must possess a Bachelor's Degree.
• Must be licensed in MN, ND or IA per requirements.
• English communication skills required.

Responsibilities

• Assess & Evaluate Patients and identify needs across physical, cognitive, communication, and swallowing domains.
• Develop & Implement Treatment Plans to address deficits and promote independence.
• Provide skilled, hands-on therapy and therapeutic activities.
• Educate patients, families, and staff on pain management, safety, and daily functioning.
• Monitor progress, document evaluations, and adjust plans as needed.
• Collaborate with physicians, nurses, therapists and other healthcare professionals for coordinated care.

Pro Rehab, an established Rehab company with over 35 years of successful business is hiring a Therapist (PT, OT, SLP) to join the team at Maple Lawn Senior Care, in Fulda, MN! We offer a competitive wage and benefits. No Productivity Requirements! New grads are welcome to apply!

Position Type: On-Call

Wage Range:

PT & OT $60 / hour

SLP $55 / hour

Location: 400 NE 7th St Fulda, MN 56131

Therapist Responsibilities:

• Assess & Evaluate Patients: Conduct comprehensive evaluations to identify physical, cognitive, communication, and/or swallowing needs based on discipline-specific scope of practice
• Develop & Implement Treatment Plans: Create and carry out individualized treatment plans and rehabilitation programs to address functional deficits and promote independence
• Provide Therapeutic Interventions: Deliver skilled, hands-on therapy and therapeutic activities, including exercises, manual techniques, adaptive strategies, assistive device training, and communication or swallowing interventions as appropriate
• Educate & Support Patients, Families, & Staff: Provide education such as but not limited to pain management, fall prevention, home safety, coping strategies, communication techniques, and strategies to improve daily functioning and quality of life
• Monitor Progress & Document Care: Track patient outcomes, document evaluations and progress, and modify treatment plans based on patient response and clinical judgment
• Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, therapists, educators, and other healthcare professionals to ensure coordinated, comprehensive patient care

Physical Therapist Qualifications:

• Must be a graduate from an Accredited Therapy Program and have a Bachelor's Degree
• Must possess current licensure for MN, ND or IA
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care
• Must be capable of performing the essential functions of the job, with or without reasonable accommodations
• Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required
