Responsibilities

• Provide counseling sessions to assigned patients.
• Complete all required documentation accurately and timely.
• Perform initial assessment and ongoing care.
• Develop individualized care plans with multidisciplinary team.
• Address co-occurring disorders.
• Work as a member of the multi-disciplinary treatment team.
• Coordinate with Case Managers and patient support staff.
• Connect patients to internal/external resources as appropriate.
• Perform other duties as assigned.

Duties and Responsibilities

• Provide counseling sessions to assigned patients

• Performs initial assessment and on‑going care
• Participates in the development of individualized care plans
• Addresses co‑occurring disorders
• Work as a member of the multi‑disciplinary treatment team
• Coordinate with Case Managers and patient support staff

• Performs other duties as assigned

Requirements

• Must have an active registration in good standing with the State of California as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselors (LPCC) or Clinical Psychologist.

Work Environment

FHCSD's services are all outpatient and designed for those struggling with mild to moderate acuity levels. You will have support from other therapists, case managers, a scheduling team, administrators, and a wide range of other medical professionals in our facilities. The Mental Health Department strives to create an inclusive community that fosters teamwork and continuous learning.
