What you’ll do

• Act as mental health support for those referred and/or interested in these services.
• Message patients about behavioral health questions through the Diana Health platform.
• Conduct in-office and telehealth mental health visits via warm hand-off process from providers and/or patient request.
• Provide ongoing mental health treatment and refer out, as needed.
• Complete timely clinical documentation within our EHR system.
• Collaborate with other providers to deliver excellent patient care, including:
• Working with CNM/OB-GYN on plans of care and escalation needs, including medication management.
• Collaborating with Health Coaches regarding patient wellness.
• Partnering with Care Navigators to provide additional support.
• Serve as the office expert on behavioral health by:
• Providing staff training on behavioral health and workflows.
• Leading expert talks and support groups relevant to behavioral health.
• Connecting patients and providers to external referrals and resources as appropriate.

Qualifications

• Licensed Clinical Social Worker (LCSW) or Marriage Family Therapist (MFT) with current Florida licensure.
• At least 2 years of experience providing mental health support to a similar client panel.
• Excellent communication, interpersonal, and organizational skills.
• Strong computer skills and familiarity with EMRs.
