Job summary

Fallon Health is seeking a Therapeutic Recreation professional to join the Summit ElderCare PACE program. The role focuses on assessing needs and crafting individualized recreational plans to promote mental health, social connections, and physical well-being for elderly participants.

You will lead group and individual activities, contribute to monthly planning, and work within an interdisciplinary team to support participants and families while upholding Fallon Health policies.

Qualifications

• One or more years of experience in an activities program in a health care setting serving frail or elderly people in the last 5 years.
• CPR and Alzheimer's certifications, or willingness to be certified.
• Access to reliable transportation to travel throughout the service area.

Responsibilities

• Conduct large and small therapeutic recreational groups and individual activities to address cognitive, physical, social, psychosocial, spiritual and artistic needs.
• Contribute ideas for monthly activity planning calendars.
• Provide education and culturally competent activities, including exercise, arts and crafts, music, and spirituality.
• Ensure participant choices and engagement when planning activities.
• Foster a warm social atmosphere and support participants with dignity and respect.
• Maintain cleanliness of supplies and report damaged equipment.
• Document attendance and participation daily as assigned.
• Participate in interdisciplinary team meetings and case conferences.
• Communicate changes in participant condition or complaints to supervisor.
• Complete tasks reliably within assigned timeframes.
