Licensed Practical Nurse - Full Time - 2-10 PM Shift-Every Other Weekend

The Addison of Moorehead Place

We are seeking a dedicated Licensed Practical Nurse for a full-time position working the 2-10 PM shift, every other weekend.

Responsibilities
  • Administer medications to residents in compliance with federal, state regulations, and company guidelines.
  • Assist with admissions, including assessments and documentation for new residents.
  • Develop and update acute and long-term care or service plans.
  • Track clinical information such as wounds or infection control under the direction of the director.
  • Provide health-related consultation within the community.
  • Notify physicians and families of changes in resident health, ensuring proper documentation.
  • Respond promptly to emergencies.
  • Maintain communication with all departments and staff.
  • Conduct educational meetings for residents and staff.
  • Ensure confidentiality of resident information.
  • Handle discharges, assessments, and clinical documentation as needed.
Qualifications
  • Completed training from an accredited nursing program.
  • General nursing knowledge with at least 1+ years of experience, preferably in senior care or related fields.
  • Current Licensed Practical Nurse’s License in the practicing state.
  • CPR certification.
  • Proficiency in English for communication and emergency response.
  • Basic computer skills.
