Job summary

MEDStreamline, Inc. is seeking an onsite Licensed Practical Nurse (LPN) to provide clinical support across assigned long-term care and senior living facilities.

The role requires daily travel within the territory, documenting patient data, and collaborating with facility staff to deliver proactive patient care. Qualifications

• Active, unrestricted LPN license in the state of employment.
• Minimum one year of nursing experience in a clinical setting.
• Ability to travel daily within assigned territory.
• Strong interpersonal and customer service skills.
• Must reside within assigned territory.

Responsibilities

• Perform resident assessments and document vital signs using MedStreamline's monitoring devices.
• Accurately document patient information according to company standards.
• Provide onsite clinical support across assigned facilities.
• Identify and escalate clinical concerns to appropriate staff.
• Support facility staff and care coordination initiatives.
• Assist with device education and troubleshooting.
