Job summary

LilyCare Hospice in Bedford, New Hampshire seeks a Hospice Community Liaison to build and expand our southern New Hampshire referral network. This field role covers the assigned territory with daily travel and direct collaboration with hospitals, skilled nursing facilities, and physician practices.

You will educate referral sources on hospice eligibility, the Medicare hospice benefit, and LilyCare’s model of care.

Qualifications

• Two or more years of hospice or home health business development preferred.
• Established referral relationships in southern New Hampshire strongly preferred.
• Valid driver's license, reliable vehicle, and current auto insurance.

Responsibilities

• Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices across the assigned territory.
• Educate referral sources on hospice eligibility, the Medicare hospice benefit, and the LilyCare model of care.
• Develop and execute a quarterly territory plan with defined target accounts and measurable referral goals.
• Plan and deliver in-services and community education events, managing the associated marketing budget.
