Description

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Coordinate day-to-day on-site services for tenants, managing a caseload of approximately 20-25 households, ensuring services align with RiseBoro’s mission.
  2. Conduct needs assessments and develop tenant-centered support plans, reviewing and updating them periodically.
  3. Provide case management, assistance, advocacy, and referrals to community services as needed.
  4. Assist tenants in maintaining housing subsidies and benefits required for permanent housing.
  5. Support tenants in obtaining and maintaining government benefits, advocating as necessary.
  6. Facilitate tenant engagement through workshops and social activities, including translation services as needed.
  7. Educate residents about on-site services and community resources.
  8. Maintain up-to-date records, including personal, eligibility, service, and correspondence files.
  9. Conduct monitoring visits, telephone reassurance calls, and periodic inspections with property management.
  10. Collaborate with community partners to support aging in place, including coordinating home care and discharge planning.
  11. Communicate with property management regarding resident issues, incidents, and hospitalizations during off-hours.
  12. Participate in case conferences and coordinate with program staff for tenant success.
  13. Monitor recreational activities such as art, yoga, and gardening sessions.
  14. Perform additional responsibilities as the position evolves.

EDUCATION AND EXPERIENCE
