At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.
Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.
The Temporary Referral Coordinator plays a crucial role in ensuring the smooth operation of tasks related to patient data management, communication, tracking, and specialty referrals. Responsibilities include meticulous data entry, responding to team inquiries, proactive tracking, and involvement in special projects as assigned.
This position is remote for a 3-4 month contract, M-F shift 8am-5pm; familiarity with the Inland Empire region (San Bernardino or Riverside Counties) is preferred.
