Responsibilities

• Provide supervision and support to clients, including identifying housing-related needs and setting goals.
• Assisting with transportation, recreational activities, and medical or psychiatric appointments
• Supporting clients in learning and implementing daily living skills to enhance independence
• Delivering case management services to help clients sustain benefits and housing
• Consult with clinical staff regarding client concerns and requirements
• Conduct therapeutic rehabilitative skill development
• Contribute to intake assessment, needs assessments and housing support plans
• Review support plans collaboratively with clients
