HomeFirst is seeking a Case Manager for the Cold Weather Shelter Program in Mountain View. This temporary, full-time, non-exempt role focuses on ending homelessness via Housing First and Harm Reduction.

The role requires providing direct services, documentation, and collaboration with partners, with a schedule Monday–Friday in Mountain View during winter operations.

Qualifications

  • Minimum two years’ experience in social services.
  • No experience required for applicants with a bachelor’s degree in psychology, counseling, mental health, human services or related field.
  • Lived experience OR understanding of and sensitivity to the needs of the unhoused population.
  • Fluent in Microsoft Office Suite.

Responsibilities

  • Provide case management services including enrollment, safety support, needs assessments, budgeting, information and referral, crisis de-escalation, safety planning, and advocacy.
  • Documentation of case management activities in HMIS within 3 business days and maintain paperwork.
  • Engage with community partners to coordinate support services.
  • Attend meetings and trainings relevant to the position.

The Case Manager is committed to ending homelessness via the principles of Housing First and Harm Reduction. They demonstrate this commitment through their proactive and creative approach to program interventions and services. They recognize the time sensitivity required to support those in crisis to maintain or secure housing stability and exercise empathy toward those we serve. They apply this same passion in their care and attention for the details, contract compliance, and pursuit of performance benchmarks.REPORTING RELATIONSHIPSThis role reports to the Program Manager.PRIMARY RESPONSIBILITIESDirect Services (70%)Provide case management services. This may include:Initial enrollmentSecurity and safety support of participantsOngoing needs and status assessmentsBudgetingInformation and referralCrisis de-escalationSafety planning, risk assessment, and mandated reportingEmployment and benefits assistanceAdvocacyOther direct service provisionCollaborate with community partners regarding support services for the participantsMaintain open lines of communication, and provide regular updates to supervisorEngage in the consistent application of evidence-based practicesMaintain professional relations and conductMeet regularly with program participants at a frequency determined by the participant's needsDocumentation (20%)Document, via progress notes in HMIS, all case management activity within 3 business days and maintain all other case file paperwork and documentationParticipate in ongoing analysis of, and program alignment with, industry best practicesProactively and consistently improve the quality and effectiveness of program servicesConduct and document ongoing re-assessments of the participant's needs and status, to include VI-SPDATGeneral (10%)Attend all meetings and training relevant to this positionAssist with office-related tasks as neededAssist with other duties assignedBENEFITSHomeFirst provides comprehensive benefits, including 100% employer-paid medical, dental, and vision coverage and additional wellness options like chiropractic care, mental health support, pet insurance, and an Employee Assistance Program.
