Job summary

Eventus WholeHealth is seeking a clinical support professional to help providers focus on patient care and triage. The role serves adults in skilled nursing facilities, assisted living, and private residences through a network of clinicians and staff.

Responsibilities include creating encounter notes, reconciling medications, updating labs/imaging, and notifying PCPs to ensure coordinated care. CMA is a plus; strong typing skills preferred; remote work option available.

Qualifications

• High School Diploma or equivalency required or equivalent.
• Certified Medical Assistant degree is a plus.
• Typing speed of 45wpm or greater.
• Medical Scribing experience preferred.

• Ensure that all clinical providers are able to focus on triaging and taking care of patients
• Prepareclinical documentation for the provider as requested
• Completebasic opening of encounternotefor acute Tele Triage visits
• Perform medication reconciliation on patientswho are being seen for acute Tele Triage visits
• Enter lab and imaging results into patient’s chart
• Document appropriate triage note encounter and timeforCCMservices
• Send updates to PCPs for patients that are triaged
• Make phone calls to facilities to gather information for provider

Patient Support

• Check all forms of communicationseveral times daily to ensure patient needs are met. Primarily will be using Telemediq.
• Inform patientand family ofrecent visits, new orders and defer to provider when needed
• Communicate transitions of care to Tele Triage team to enhancepatient care and education

Qualifications/Education

• High School Diploma or equivalency
• Certified Medical Assistant degree a plus
• Typing speed of 45wpm or greater
• Medical Scribing experience preferred
