Job description

Join to apply for the Telephonic Nurse Case Manager II role at Elevance Health.

Position Details:

  • Location: Virtual - enabling full-time remote work with required in-person training sessions. This promotes flexibility, productivity, and work-life balance.
  • Hours: Monday - Friday, 9:00am to 5:30pm EST, with one late evening shift from 11:30am to 8:00pm EST.

Additional Requirements:

  • Multi-State Licensure required as service will be provided across different states.
  • Online pre-employment skills assessment required, to be completed within 48 hours of receipt.

Job Responsibilities:

  • Perform care management for members with complex and chronic needs via telephonic assessments, care plan development, coordination, and evaluation.
  • Ensure access to appropriate services, facilitate authorizations/referrals, coordinate resources, and monitor care effectiveness.
  • Collaborate with Medical Directors and Physician Advisors on treatment plans.

Minimum Requirements:

  • BA/BS in health-related field and 5 years of clinical experience, or equivalent education and experience.
  • Current RN license in applicable state(s).

Preferred Skills and Experience:

  • Case Management certification, telephonic case management experience, and managed care experience.
  • Proficiency with Microsoft Office and ability to learn new systems quickly.
  • Minimum 2 years' experience in acute care and telephonic case management preferred.

Compensation: Salary range $76,944 to $120,912, based on location and experience. Benefits include comprehensive health plans, 401(k), stock purchase, and more.
