Job summary

Kroger Health in Ohio is seeking a pharmacist to provide patient-centered services from a centralized contact center via telephone. Responsibilities include comprehensive medication reviews, targeted interventions, adherence monitoring, and enhanced patient education to improve clinical and economic outcomes.

You will work with pharmacies, providers, and patients to identify and resolve medication-related problems, set therapeutic goals, and communicate recommendations clearly.

Qualifications

• Pharmacy degree required and licensed in resident state.
• ASHP-accredited residency or 2+ years pharmacy experience.
• Strong written and oral communication to patients and providers.
• Willing to obtain nonresident licenses as assigned.

Responsibilities

• Identify and resolve medication-related problems and arrange follow-up care.
• Help patients identify adherence barriers and develop solutions.
• Communicate effectively with patients, pharmacies, and providers.
• Formulate therapeutic goals using guidelines and primary literature.
• Make professional recommendations to the healthcare team.
