Qualifications

• 2+ years in a health-related field
• 2+ years of customer service experience
• Proficient in Microsoft Office Suite and ability to use these tools in Care Manager Specialist role
• Access to private, dedicated workspace
• Dependents must have separate care arrangements during work hours

Responsibilities

• Telephonic engagement with members (50–75% of day) to coordinate care
• Outreach to hard-to-reach members
• Develop individualized care plans with members
• Document care management activities in EHR
• Collaborate with interdisciplinary care team to address barriers
• Identify and connect members with health plan benefits and community resources
• Ensure regulatory compliance within timelines
• Consult with Care Manager RN on clinical knowledge and medications
• Leverage RN expertise to address member needs
• Support team objectives and process improvements
