In this role, you will connect with members and their families over the phone to assess their needs and help guide them toward resources that support their health and wellbeing. The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.

  • Perform all duties in alignment with the DHS contract for Medicaid Managed Care Organizations, specifically regarding the Wisconsin Interdisciplinary Care Team (WICT).
  • Tailor assessments and interventions according to the member’s level of engagement, activation and with consideration of the member’s priorities.
  • Conduct transitions of care activities to reduce readmissions.
  • Actively problem solves with staff, members and providers in finding alternatives or solutions to their medical, behavioral health and social needs
  • Assists with the oversight of care plans and prioritization of complex medical, behavioral health and social needs.
  • Recognizes and acts in accordance with the nursing scope of practice and role within the interdisciplinary team. Concurrently, have an understanding and appreciation of the role and scope of practice of the other disciplines on the team to maximize effective collaboration in meeting the member’s needs.
  • Participates and contributes to regular member staffings to keep the team informed of updates to the care plan, collectively problem solve and determine the enrollment/graduation of members.
  • Serves as a consultative resource for other care management staff.
  • Provides accurate and timely documentation per work processes and guidelines

Required Qualifications
  • Bachelor's degree
  • 3 - 5 years of clinical acute care and/or care management experience
  • Reside within the State of WI
  • Licensed Registered Nurse (RN) in the WI with no disciplinary action
  • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Preferred Qualifications
  • Experience with discharge planning and patient education for adult acute care
  • Managed care experience
  • Certified Case Manager (CCM)
Additional Information:
  • Workstyle: Home- Associates Home office environment is their primary work location, but they will use Humana office space on an as needed basis for collaboration and other face-to-face needs.

  • Travel: 5-10% as needed

  • Typical Workdays/Hours: Monday – Friday, 8:30 am – 5:00 pm CST

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$71,100 - $97,800 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
