Humana Inc. is seeking a Care Coach (FIDE) to support members’ health and wellness in Illinois. You will conduct comprehensive assessments, identify needs, and guide members and families to essential resources for optimal care.

This remote role involves telephonic care management, monitoring progress, and maintaining care plans to achieve positive outcomes. Qualified candidates will have a bachelor's degree in health or related field or LPN licensure, at least 2 years of case management
