St. Paul’s PACE Corp in San Diego, CA, seeks a full-time Triage Registered Nurse (RN) for the PACE program. The role involves direct patient care, collaboration with the IDT, and coordination with health care providers across settings. Typical schedule is Monday–Friday, 8:00 AM–4:30 PM.
The RN must hold a CA license with BSN or higher preferred. Minimum one year RN experience; PACE/clinic/hospice/home health and telephone triage experience are preferred. Travel to multiple sites is required.

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a Full-time position with a typical schedule of Monday - Friday, 8:00AM - 4:30PM.
Job Summary: The Triage Registered Nurse (RN) is responsible for providing nursing services to assigned patients. RNs provide direct patient care, collaborate with other Interdisciplinary Team (IDT) members within the care team, and with other health care providers across various care settings.
Patient care provided by RNs is delivered in a manner that promotes an interdisciplinary team approach, respects patients' and families' goals for care, and meets Medicare/Medicaid conditions of participation and accreditation requirements. The RN practices in alignment with the organization’s mission, values, and clinical philosophy. Upon request, participates in all areas of clinical services.
