Qualifications

Responsibilities

• Install and remove telemonitoring equipment in patients' homes across the state.
• Provide post-installation follow-up with patients.
• Coordinate follow-up with telemonitor vendors and staff after installation.
• Assist with related paperwork.
• Communicate clearly with patients, considering age, culture, needs, and abilities.
• Ensure punctuality, proper time-off usage, and comply with annual education/medical requirements.
• Perform other duties as assigned.

Essential Duties and Responsibilities

• Responsible for the installation and removal of Telemonitor equipment in patient’s homes throughout the state.
• Provides follow-up to patients after installation.
• Provides follow-up to telemonitor vendor and telemonitor staff after installation.
• Assists with associated paperwork.
• Communicates effectively and tactfully with patients, recognizing their age, cultural diversity, needs, abilities and physical condition.
• Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
• Performs other duties as required.

Minimum Qualifications

Education: Minimum of High School Diploma; Associate preferred

Experience: Minimum of Two (2) Years’ Experience in the geriatric field

Certificates, Licenses, Registrations: Maintains current Driver’s License and auto insurance
