Responsibilities

• Educate patients and families about health conditions, treatments, and post-discharge care.
• Monitor and record vital signs and observe changes in patient conditions.
• Address complex health situations with clinical judgment and document care per standards.
• Collaborate with the medical team as needed and ensure accurate charting.
