Job summary

New Mexico VA Health Care System is seeking a Nursing Assistant for the 5D Telemetry Acute Care Service. The NA will perform a range of direct patient care activities under supervision, monitor vitals and glucose, observe patient status, and report changes promptly to the care team.

Responsibilities include assisting with daily living activities, documenting care and observations, and participating in treatment planning while ensuring patient safety and comfort within a telemetry acute care

Responsibilities

• Utilizes glucose monitoring machine to receive glucose readings for veterans; refers deviations to appropriate health care staff.
• Provides direct care for complex veterans and makes observations of subtle changes in condition (behavior, mood, skin color, appetite).
• Documents care given, observations, and responses on appropriate tools or forms.
• Responds to subtle patient changes by contacting nursing staff; assists with basic procedures and activities of daily living.
• Participates in Treatment Planning Meetings and uses equipment to promote self-care (lift equipment, hydrotherapy tubs).
