Job summary

Aurora Health Care Metro, Inc. is seeking an experienced Registered Nurse to provide holistic patient care on a 23-bed telemetry floor. You will assess needs, implement interventions, and collaborate with a hospitalist team to ensure high quality outcomes.

Responsibilities include discharge planning, evidence-based practice, and continuous learning. Requires BLS certification and completion of a nursing program; typically 1 year of clinical nursing experience is expected.

Responsibilities

• Uses the nursing process to assess the needs of the patient, plan and implement individualized interventions and evaluate the effectiveness of the plan of care.
• Anticipates and recognizes significant changes in the patient’s condition, status, and need for care; coordinates with care team.
• Implements strategies to reduce patient risk and increase patient safety; educates patients and families.
• Coordinates discharge planning using interdisciplinary approach and applies evidence-based practice.
• Provides ongoing evaluation of patient, team, and unit outcomes and supports governance initiatives.
• Promotes effective communication and collaboration among healthcare team members; acts as a leader within the care team.
