Key Responsibilities

• Perform telemedicine visits to assess stroke survivors for rehabilitation device eligibility and write prescriptions as needed
• Determine appropriateness for Principal Illness Navigation services and develop disease-specific care plans
• Refer patients to internal and external supportive services while maintaining timely documentation

Required Qualifications
