The Therapist Telehealth is a virtual, 100% fully remote position. The Therapist develops, coordinates, and facilitates all direct social services to patients and their families assigned and performs duties as directed by their manager. The Therapist will work in a virtual setting that includes partial hospitalization and intensive outpatient services.
This position is fully remote; however, the selected candidate must reside in a state that Rogers currently operates in: WI, MN, IL, PA, TN, FL, GA, CO, CA, WA.
Job Duties & Responsibilities:
- Provide social services to patients and their families/significant others virtually, including therapeutic treatment services to individuals, groups, families, and significant others.
- May provide individual treatment sessions based on program and patient needs.
- Provide crisis intervention services as needed and assess safety daily.
- Interact with patients, family members, significant others, and referents with respect and dignity.
- Participate in psychoeducational support sessions with patients and families.
- Provide case management and therapeutic interventions to all assigned patients.
- Serve as patient advocate in case of grievances, following System policy and procedure.
- Adhere to provided materials to ensure clinical effectiveness and consistency, manage expectations, and coordinate with referring providers and care team.
- Implement goals and interventions as specified in the Care Plan, ensuring they are SMART and patient-specific.
- Document progress according to the treatment plan.
- Troubleshoot technology issues during programming with patients.
- Complete admissions documentation, including assessments, safety plans, initial treatment goals, and necessary consents.
- Maintain ongoing treatment documentation, including progress notes, updates to the Care Plan, and contact notes.
- Complete discharge documentation and coordinate aftercare with stakeholders.
- Report and document suspected cases of abuse or neglect according to law and policy.
- Provide leadership and guidance in clinical interventions, evaluate outcomes, and participate in interdisciplinary staffing.
- Communicate professionally and accurately with team members and external departments, reporting pertinent data and addressing issues promptly.
- Demonstrate a strong interdisciplinary approach, fostering effective communication and teamwork.
- Conduct self professionally, demonstrating organizational skills, respectful communication, and leadership.
- Support staffing and scheduling changes as needed, demonstrating flexibility and commitment.
- Complete orientation, competencies, training, and documentation requirements.
- Participate in staff meetings, projects, and initiatives to improve skills and organizational effectiveness.
- Comply with organizational policies, standards of practice, and ethical guidelines.
- Participate in site committees and ROS (Rogers Operating System) initiatives, applying continuous improvement principles.
- Ensure patient safety by intervening appropriately, communicating safety concerns, and following emergency protocols.
- Meet educational and licensure requirements, maintaining all obligations related to licensing and program regulations.
- Promote the organization’s mission and goals, representing Rogers professionally in all interactions.
- Perform other duties as assigned, contributing to a positive and effective work environment.
