Job summary

MetaPhy Health, Inc. is seeking a Telehealth Care Coordinator (LPN) to educate, support, and enroll patients through our telehealth platform. You will enroll a minimum of three patients per day into the chronic care management program and document interactions in the EHR.

The role is fully remote with a Monday–Friday day schedule and potential Friday off upon meeting weekly goals. Competitive benefits include PTO, holidays, and health insurance.

Qualifications

• Active LPN license with multi-state or compact eligibility
• 1–3 years experience with Microsoft 365, EMR/EHR, Windows and PC hardware
• 1–2 years healthcare setting experience preferred
• Legally eligible to work in the United States
• High-speed internet required

Responsibilities

• Provide patient education, support, and care via telehealth platform in alignment with physician practices
• Educate patients on enrolling into chronic care program and set program expectations
• Coordinate information between providers, clinical staff, and patients
• Manage and update patient-centered care plans with the healthcare team

Telehealth Sales & Enrollment Nurse (LPN) – Remote

General Information:

As a Telehealth Care Coordinator at MetaPhy, you will play a crucial role in delivering patient education, support, and care through our telehealth platform. You will be responsible for enrolling a minimum of three (3) patients per day into our chronic care management program. This is a performance-based role: meeting the daily enrollment quota is a core expectation, regardless of how many outreach calls or follow-ups are required to achieve it.

Success in this role depends on persistence, effective communication, and a proactive approach to patient engagement. Your responsibilities will include:

Serve as a liaison between providers, clinical staff, and patients

• Conducting patient outreach and enrollment
• Documenting interactions and observations in the electronic health record (EHR)
• Providing nutritional guidance and education
• Prioritizing tasks effectively in a dynamic, fast-paced environment

What We Offer:

• This position offers a base pay of $22 per hour, with commission opportunities that typically boost earnings to approximately $25 per hour on average.

Key Responsibilities:

• Provide patient education, support, and care via telehealth platform in alignment with physician practices.

• Coordinate information between providers, clinical staff, and patients to ensure comprehensive care delivery.
• Manage and update patient-centered care plans in collaboration with healthcare team members.
• Document clinical observations, updates, and patient education in EHR.
• Schedule, confirm, and coordinate virtual patient encounters.
• Demonstrate professionalism, empathy, and effective communication skills with patients and colleagues.
• Efficiently oversee and enroll 3 patients per day into our chronic care management program.
• Prioritize tasks efficiently and work effectively in a dynamic environment.

Qualifications:

• Highspeed reliable internet is required.
• Licensed Practical Nurse (LPN) with an active multi-state or compact license.
• 1-3 years of experience with Microsoft 365, EMR/EHR, Windows operating system, and laptop/desktop professional experience (required).
• 1-2 years of experience in a healthcare setting (preferred).
• Interest in Chronic Care Management delivered through telehealth platforms.
• Legally authorized to work in the United States.
