Job summary

CareMed Group seeks a Director of Telehealth to lead a restructured telehealth program across multiple states, oversee CCM and BHI initiatives, and drive clinical team performance for after-hours and weekend coverage in long‑term care facilities.

The role requires active NP/PA licensure in at least one operating state, 5+ years in clinical settings with leadership experience, and proficiency with CMS billing for CCM/BHI. Remote/hybrid work with periodic on-site visits is expected.

Qualifications

• Active NP or PA license in at least one CareMed operating state (OK, KY, WV, OH).
• Minimum 2–3 years in clinical leadership or program management.
• Direct experience with telehealth service delivery in skilled nursing or similar settings.

Responsibilities

• Own end-to-end build-out and ongoing management of the telehealth program, including the clinical rotation schedule.
• Oversee CCM and BHI program build: eligibility screening, enrollment workflows, and patient consent.
• Monitor documentation standards, billing accuracy, and CMS payer requirements for CCM and BHI.
• Direct supervision of clinicians: scheduling, coaching, performance management, and escalation support.
• Maintain liaison with facilities and executive leadership on program performance.
