Wider Circle, a mission-driven healthcare organization, seeks a licensed physician to serve as a consulting clinician for an early-stage care program. You will conduct initial virtual visits to understand medical needs, barriers to care, and health goals, guiding the member's care journey with a multidisciplinary team.

The role emphasizes innovative care delivery, telehealth, and population health models beyond traditional care, with a flexible, collaborative, startup-like environment.

Qualifikationen

• MD or DO with an active, unrestricted license; multiple state licenses strongly preferred.
• Board certified or board eligible in Family Medicine, Internal Medicine, or related primary care specialty.
• Experience with Medicare/Medicaid and complex populations is preferred.

Aufgaben

• Conduct initial telehealth visits with enrolled members.
• Assess medical needs, care gaps, and barriers to accessing care.
• Establish a clear clinical plan and care priorities.
• Collaborate with care team members to ensure a smooth handoff and continuity of care.
• Provide clinical insight to help refine program workflows during the pilot phase.
• Support a patient-centered approach addressing social and medical needs together.
