Job summary

Sprenger Therapy is seeking a licensed Physical Therapist to provide remote telehealth services to residents in our skilled nursing communities. You will work with an interdisciplinary team to deliver resident-centered care via secure video technology.

The role emphasizes clinical autonomy, collaboration with on-site staff, and adherence to Ohio licensure requirements while supporting mobility, safety, and functional goals.

Qualifications

• Ohio license or eligibility to practice physical therapy in Ohio.
• PT degree from an accredited program (B/P/M/DPT).
• Experience in geriatrics, skilled nursing, or telehealth preferred.
• Strong communication and rapport-building through video.
• Ability to work independently with clinical judgment.
• Team-oriented mindset and collaboration with on-site staff.

Responsibilities

• Complete physical therapy evaluations and reassessments via secure telehealth.
• Develop and adjust individualized plans of care with residents and families.
• Provide telehealth treatment focusing on mobility, strength, balance, safety, and independence.
• Collaborate with on-site therapy staff during evaluations and treatments.
• Supervise and direct Physical Therapist Assistants as allowed by law.
• Participate in care planning and discharge planning.
• Conduct screenings to identify residents who need PT intervention.
• Document services clearly and efficiently using electronic systems.
