Job summary

Sprenger Therapy is the in‑house rehabilitation team for Sprenger Health Care Systems, a family‑owned senior living organization. Our telehealth therapists work with on‑site teams to deliver connected, individualized care to residents in our Ohio communities.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Current Ohio PT license or eligibility.
• Experience in skilled nursing, geriatrics, post-acute care, or telehealth preferred.
• Strong clinical judgment and ability to work independently.
• Clear communication skills and ability to build rapport through video.
• Team-oriented mindset and ability to collaborate with on-site staff.
• Comfort using electronic documentation and video technology.
• Reliable broadband and private workspace for confidential care.
• Commitment to patient-first, outcomes-driven care.

Responsibilities

• Complete physical therapy evaluations and reassessments through secure telehealth technology.
• Develop individualized plans of care with residents, families, and the care team.
• Provide skilled telehealth treatment focused on mobility, strength, balance, safety, function, and independence.
• Collaborate with on-site therapy staff during evaluations, treatments, and assessments.
• Supervise and provide clinical direction to Physical Therapist Assistants as permitted by law.
• Participate in care planning and discharge planning.
• Screen residents and identify those who may benefit from PT intervention.
• Communicate with residents, families, nursing staff, physicians, and care team members.
• Document services clearly, accurately, and efficiently.
• Create a telehealth experience that remains personal and resident-centered.

Sprenger Therapy is seeking an Ohio-licensed Physical Therapist to provide telehealth services to residents within our skilled nursing communities. You’ll deliver meaningful, patient-centered care through secure video technology while collaborating closely with our on-site therapy teams.

What You’ll Do

• Provide skilled telehealth treatment focused on mobility, strength, balance, safety, function, and independence
• Collaborate with on‑site therapy staff during evaluations, treatments, measurements, and functional assessments
• Supervise and provide clinical direction to Physical Therapist Assistants, as permitted by Ohio law and payer requirements
• Participate in care planning and discharge planning
• Conduct appropriate screenings and identify residents who may benefit from physical therapy intervention
• Communicate with residents, families, nursing staff, physicians, and other members of the care team
• Document services clearly, accurately, and efficiently
• Create a telehealth experience that remains personal, engaging, and resident-centered

What We’re Looking For

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program
• Current Ohio Physical Therapist license or eligibility for Ohio licensure
• Experience in skilled nursing, geriatrics, post-acute care, or telehealth preferred
• Strong clinical judgment and ability to work independently
• Clear communication skills and the ability to build rapport through video
• Team‑oriented mindset and ability to collaborate effectively with on‑site staff
• Comfort using electronic documentation and video technology
• Reliable high‑speed internet and a private workspace appropriate for confidential healthcare communication
• Commitment to patient‑first, outcomes‑driven care

Experienced clinicians are preferred. New graduates with strong geriatric clinical experience and the ability to practice independently may also be considered.
