Base pay range

$75,000.00/yr - $140,000.00/yr

Must have 4 or more active NP licenses in the following states:

PA, NY, NJ, FL, CT, MA, NH, NM, VT, TX, WV

Position Summary

The Nurse Practitioner (NP) will work in collaboration with the clinical interdisciplinary team to coordinate and support the participant’s functional, clinical, and psychosocial needs. This role requires conducting telehealth visits via video to provide quality care for transitional care management (TCM) and chronic care management (CCM) for geriatric patients through remote patient monitoring (RPM).

Job Responsibilities

1. Conduct virtual/remote video visits with patients.
2. Review patient’s medical history and formulate a comprehensive diagnostic list using clinical knowledge and judgment, ensuring accurate documentation in the medical record.
3. Assess current symptoms, manage active diagnoses, and identify chronic conditions for ongoing management.
4. Prescribe medications and provide insights into medication management.
5. Collaborate with case managers for labs, diagnostics, DME, and other recommendations.
6. Review change in condition reports and schedule follow-up visits as needed.
7. Complete and submit daily billing logs with CPT and ICD 10 codes.
8. Participate in interdisciplinary team meetings as needed.
9. Ensure compliance with HIPAA regulations and maintain PHI security.
10. Contribute to team efforts by accomplishing related tasks as needed.
11. Perform other duties as assigned by supervisor.

Required Experience, Education, Skills & Abilities

1. Current and valid RN and Nurse Practitioner licenses in the practicing state.
2. Minimum of three years of clinical nursing experience in home care, skilled nursing, or hospital settings.
3. Over 3500 hours of NP experience, with ability to bill Medicare and Medicaid without restrictions.
4. Proficiency in electronic documentation and HER platforms.
5. Experience with Microsoft Office and technology-based healthcare platforms.
6. Excellent customer service, interpersonal, and communication skills; bilingual in English/Spanish preferred.

9. Effective organizational and time management skills.
