Job summary

Holon Health is seeking an experienced Nurse Practitioner in Washington to support individuals with SUD through integrated, collaborative care. This role offers remote flexibility and can be structured as full-time or part-time, with telehealth-based assessments and care coordination to support clients on their recovery journey.

Qualifications

• Licensed and experienced Nurse Practitioner with active WA NP license and DEA license.
• Experience in addiction medicine or psychiatry and commitment to person-centered care.
• Strong collaboration within integrated care teams and ability to work remotely.

Responsibilities

• Conduct telehealth-based health evaluations and medical management for SUD clients.
• Diagnose conditions and create individualized treatment plans.
• Prescribe and manage medications, monitor side effects, and adjust as needed.
• Collaborate with care team and external providers for coordinated, whole-person care.
• Serve as a clinical consultant on high-need cases and complex workflows.
• Educate clients and families on treatment plans, medications, and recovery supports.
• Maintain accurate documentation compliant with ethical and medical standards.
• Participate in team meetings and ongoing professional development.
