Abode Care Partners, affiliated with BrightSpring Health Services, seeks an on-site Licensed Practical Nurse (LPN) to support telehealth visits across Jacksonville, FL and surrounding communities. You will assist providers, manage device connections, and help obtain patient vitals during virtual encounters.

The role requires an LPN license and flexibility to travel within the market area, with emphasis on privacy and HIPAA compliance.

Qualifications

• Licensed Practical Nurse (LPN) required
• Experience with telehealth platforms or mobile health technology preferred
• Must be open to flexible scheduling based on provider and community needs
• Strong interpersonal and communication skills with a patient-centered approach
• Ability to work independently and adapt to varying schedules and environments
• Reliable transportation and willingness to travel to multiple locations within the business market area

Responsabilités

• Facilitate telehealth visits by setting up and managing iPad or mobile device connections between patients and providers
• Remain present during telehealth visits to assist with communication, positioning, and any real-time needs
• Obtain and accurately document basic patient vital signs, including blood pressure, temperature, pulse, and other requested measurements
• Support providers by relaying clinical information as needed during virtual visits
• Ensure all equipment is functioning properly prior to each appointment
• Maintain patient privacy and confidentiality in compliance with HIPAA regulations
• Communicate effectively with facility staff, providers, and internal team members to coordinate care
• Assist with light administrative or clinical tasks as needed to support patient care operations
