Overview

SNF Triage LPN (Fully Remote)

Part-Time: 2 positions available immediately

Updated Available Schedule:

• Friday, Saturday & Sunday 10am-6pm CST
• Thursday, Friday, Saturday 10pm-6am CST

This LPN provides On-Call and Triage services for the assigned population, following triage protocols. Responsibilities include developing and implementing care plans, assessing health status, performing chart reviews, and data analysis.

Responsibilities

• Monitor assigned phone lines to ensure timely responses
• Triaging symptom-based calls and providing recommendations
• Assess severity of symptoms and refer to appropriate care settings
• Document care recommendations and notify operations
• Coordinate services with hospitals and pharmacies
• Complete assessments, develop/update care plans, and secure physician orders
• Communicate with beneficiaries via phone, text, chat, or email
• Ensure compliance with relevant requirements
• Provide health education and participate in performance activities
• Document actions and maintain time logs
• Notify practices and leadership of encounters and operational concerns
• Participate in meetings and committees as requested

Qualifications

• Degree/certification from an accredited Nursing school
• At least one year of nursing experience
• Current LPN licensure in the state of practice, with a compact license preferred
• Experience with individuals with disabilities, home health, or hospice care
