Job description

Job Summary:

Provides remote consultative and supportive nursing care to critically ill patients in the Intensive Care Units (ICUs) across multiple medical centers. Utilizes in-room camera and EPIC technology from a centralized hub. Collaborates with intensivists, hospitalists, MODs, nurses, respiratory therapists, and other staff to provide timely interventions aimed at achieving optimal patient outcomes. Guides, coaches, and assists the clinical team in daily critical care needs following AACN guidelines.

Essential Responsibilities:

1. Performs video monitoring and assessment of ICU patient data, conducts safety oversight rounds, and collaborates with the TCC physician in the remote hub to ensure timely consultation and intervention for ICU patients requiring immediate care.
2. Responds to ICU RNs' requests for nursing care consultation, providing real-time clinical decision support during evening and night shifts for patients with urgent conditions.
3. Participates in continuous performance improvement activities with remote interprofessional teams, leveraging technology to gather and evaluate quality metrics and reviewing critical care quality data.
4. Provides expert consultation for education, assessment, and treatment of critical care patients.
5. Maintains current knowledge of evidence-based practices and updates from organizations like the AACN.
6. Identifies opportunities for improvement, reporting findings to the Physician-in-Charge, Clinical Lead, and performance teams.
7. Monitors practice to ensure policies and procedures are followed, taking appropriate actions as needed.
8. Co-leads and participates in regional committees related to critical care.
9. Serves as a subject matter expert on critical care recommendations from organizations such as AACN and TJC.
10. Develops strategies to sustain performance improvement gains over time.
11. Supports regional committees in policy development, implementation, and dissemination.
