Job summary

Bookmark Medical is seeking a Primary Care Physician (MD/DO) for Scottsdale, AZ to join a value-based, provider-focused team. The role emphasizes high-quality patient care supported by modern technology and collaborative clinical teams.

Qualifications

MD or DO degree from an accredited medical school.
Completed residency in Family Medicine or Internal Medicine.
Board certification or eligibility in Family or Internal Medicine.
Current, unrestricted license in the state of practice or eligibility before employment.

Responsibilities

Provide comprehensive primary care services including preventive care, acute visits, and chronic disease management.
Perform patient evaluations, physical exams, and diagnostic assessments.
Diagnose and manage acute and chronic conditions with individualized treatment plans.
Order, review, and interpret diagnostic tests and lab results.
Educate patients on preventive health, disease management, and treatment options.
Collaborate with advanced practice providers, specialists, and the care team.
Maintain timely, accurate documentation in the EHR.
Follow guidelines and evidence-based practices to support quality outcomes.
