Job summary

Bookmark Medical in Chandler, AZ is seeking a Family Medicine or Internal Medicine Physician (MD/DO) to join our established primary care team. The role offers a competitive base salary with performance-based incentives and a value-based care approach that supports clinicians.

Qualifications

• MD or DO from an accredited medical school.
• Completed residency in Family Medicine or Internal Medicine.
• Board certification or board eligibility in Family Medicine or Internal Medicine.
• Current, unrestricted medical license in the state or eligibility to obtain licensure prior to employment.
• Strong clinical judgment, communication skills, and commitment to patient-centered care.
• Ability to work collaboratively in a team-based primary care setting.
• Experience using electronic health record systems.

Responsibilities

• Provide comprehensive primary care services, including preventive care, acute visits, and chronic disease management.
• Perform patient evaluations, physical examinations, and appropriate diagnostic assessments.
• Diagnose and manage acute and chronic medical conditions and develop individualized treatment plans.
• Order, review, and interpret diagnostic tests and laboratory results.
• Educate patients on preventive health, disease management, and treatment options.
• Collaborate with advanced practice providers, specialists, and other members of the care team to coordinate patient care.
• Maintain timely, accurate, and complete documentation in the electronic health record.
• Follow established clinical guidelines and evidence-based practices to support quality patient outcomes.
