Description

Thresholds is seeking a skilled, energetic, and flexible Team Leader for our Youth Transitional Living Program's case management team. This program consists of REACH, a youth transitional living program (TLP), and the Mothers Project, a TLP that serves pregnant and parenting young mothers. Both TLPs offer comprehensive support to young people between the ages of 17 and 24 who are experiencing mental health challenges through residential support, case management, and therapy services. The Team Leader is responsible for the direct supervision of all the members of the case management team to ensure quality service delivery to REACH and Mothers Project members and functions as the senior clinician or "player coach" of the team.

ESSENTIAL DUTIES & RESPONSIBILITIES
  1. Leadership and Model Practices: Assume a leadership role within the Youth Transitional Living Program's case management team. Model evidence-based practices and recovery to ensure team adherence and understanding of fidelity scales. Ensure that 60% of services are provided in the member's natural setting.
  2. Assessment and Recommendations: Gather member information, assess service needs, make treatment recommendations, identify natural resources and supports, explain options to members, and assist with the application process.
  3. Supervision: Function as a DCFS Supervisor, overseeing Case Managers who act as DCFS Case Workers.
  4. Team Oversight: Lead and oversee activities of team staff providing outreach, case management, social, vocational, educational, physical well-being, and other services.
  5. Productivity and Documentation: Meet productivity targets, complete documentation timely, monitor and coach staff on productivity measures.
  6. Member Attendance and Transitions: Monitor member attendance and occupancy, handle case assignments, transitions, and closures effectively.
  7. Record Keeping: Review all documentation, maintain quality and confidentiality of case records per standards, ensure accuracy and organization.
  8. Compliance and Court Orders: Ensure compliance with court orders, assist in court as needed, and complete court documentation timely.
  9. Reviews and Tasks: Responsible for bi-annual court reviews, participate in special tasks such as representing the agency at court hearings, supervising interns, and training staff.
  10. Staff Evaluations and Development: Conduct annual performance evaluations, develop goals, and support staff professional growth.
  11. Safety and Hiring: Assess safety in homes, participate in hiring and termination decisions.
  12. Clinical Supervision and Improvement: Follow agency standards for clinical supervision, assist with Performance Improvement Plans.
  13. Other Duties: Perform additional tasks as assigned.
EDUCATION & EXPERIENCE
  1. Master's degree in Psychology, Counseling, Social Work, or related field required.
  2. LCPC, LCSW, or Licensed Clinical Psychologist preferred.
  3. CWEL license and TCI certification required (can be obtained after hire).
  4. Experience with youth case management and DCFS involvement preferred.
  5. Supervisory experience preferred.
ADDITIONAL REQUIREMENTS
  1. Flexibility for on-call rotation and occasional evenings.
  2. Valid driver's license and daily access to a personal vehicle.
  3. Willingness to transport program participants.
