Job summary

Opportunity Builders, Inc. (OBI) seeks a Day Habilitation Team Leader to provide direct care and advocate for adults with intellectual and developmental disabilities while helping implement efficient program processes. This role focuses on PCP goals, and may involve in-agency, transportation, or community-based support.

The salary range is $20.50–$21.50 hourly. OPI emphasizes collaboration with the management team to improve services, ensure safety, and maintain quality care, while guiding DSPs

Qualifications

  • High School diploma, GED, or equivalent in work experience.
  • Valid Driver’s license, no more than 3 points on driving record and 3+ years driving experience.

  • Smart phones with internet capability and basic knowledge of the internet, apps, email and GPS.
  • Minimum of 1 years’ experience working with people with intellectual and physical disabilities(may include personal experience from having a family member or member(s) with a disability)
  • Able to perform with autonomy, as well as in a team-oriented environment.
  • Strong organizational skills and the ability to prioritize.

Responsibilities

  • To aid with ADLs to adults with intellectual and developmental disabilities as needed.
  • To identify and offer activities that are meaningful to the individual being supported, seeking input from participants and caregivers.
  • To act in a dignified, respectful manner honoring the participant’s rights and PCP.
  • Provide in-person activities and virtual activities using OB I-approved platforms.
  • Document all services accurately in ICare Manager daily.
  • Work with participants to achieve individual goals and participate in PCP.
  • Collaborate with supervisors to reflect participant preferences in PCP.
  • Participate in trainings and sign off on PCP and BP when applicable.
  • Train Direct Support Professionals about participant-specific training.
  • Drive personal and/or agency vehicles for community and/or employment transportation.
  • Cover virtual services as needed.
  • Ensure meaningful activities and engagement for participants.
  • Be a Certified Medication Technician or work toward certification.

Position Summary

Opportunity Builders, Inc. (OBI) Team Leaders are DSPs who are responsible for providing direct care and advocacy for adults diagnosed with intellectual and developmental disabilities, in addition to serving in a non-supervisory leadership capacity. This particular position ensures program compliance by working directly with the management team to help implement more effective and efficient processes. Whenever possible, the Team Leader will not be scheduled to provide direct care, to allow them to focus efforts on assigned projects. When they do provide direct care, however, they are responsible for assuring that the participants assigned to them are receiving the highest quality of care and support. This includes assisting in proactive steps to work towards achieving goals outlined within the Person-Centered Plan (PCP) and may require providing direct care/support within the agency’s facility, during transportation, or within the community. The salary range for this position is between $20.50 - $21.50 hourly.

Essential Duties
Program Compliance Responsibilities
  • To complete assigned checks to ensure that OBI provides quality services and a safe environment for participants. This would include but not limited to van checks for proper wheelchair security,van cleanliness checks, zoom participation and equipment checks, and ensuring quality checks arecomplete.
  • Work in conjunction with OBI’s management team to complete specialized tasks or projects toenhance the quality of the program being offered, to support DSPs, and to develop new processesthat benefit all of OBI.
  • Work collaboratively with all team leaders in the areas of virtual services, activity execution,community access and integration, and quality care.
  • Help to implement changes that may be needed in response to incidents that occur. Specifically,assist with implementing corrective measures outlined in OBI’s Plans of Correction.
Direct Support Responsibilities
  • To aid with ADLs to adults (participants) diagnosed with intellectual and developmentaldisabilities as needed. This could include but is not limited to assistance with eating/drinking,toileting, dressing, writing, verbal communications, sharing of information between participantand other individuals, etc.
  • To identify and offer activities that are meaningful to the individual being supported, being sureto seek participant and caregiver input to help guide services.
  • To act in a manner that is dignified, respectful, and honors the participant’s rights, preferencesand needs as determined within the person-centered plan (PCP).
  • Provide activities in-person while following all health and safety guidelines, and provide virtualactivities using secure, OBI-approved platforms (i.e., zoom, facetime, etc.).
  • Provide accurate and quality documentation of all services provided, in accordance with OBI,state, and federal regulations by entering service data into ICare Manager each day that servicesare provided.
  • Work directly with participants in helping achieve individual goals and objectives.
  • Participate in the development of the participant’s person-centered plan (PCP).
  • Work with supervisors and case coordinators to communicate the preferences of the participantsto ensure this is reflected within the person-centered plan (PCP).
  • Participate in ongoing in-service training and maintain all DDA and OBI required trainings.
  • Participate in specialized training for any assigned participant, which includes reviewing andsigning off on the person-centered plan (PCP) and behavior plan (BP) when applicable.
  • Train Direct Support Professionals about individual specific training as needed.
  • Driving your own personal vehicle and/or an agency vehicle to provide community and/oremployment transportation to the participants.
  • Cover virtual services as needed.
  • Assist with ensuring there are meaningful activities for program participants.
  • Be a Certified Medication Technician or can become a Certified Medication Technician.
Leadership Responsibilities
  • Assist with the training of DSPs assuring that DSPs understand and are comfortable with theirresponsibilities and duties. This includes but not limited to ratio training, PCP, mentoring, etc.
  • Aid the program Manager and Director as needed.
Non-Essential Duties
  • Attend external meetings, conferences and training as needed/requested by supervisor.
  • Additional duties as assigned by the Director.

Physical Requirements

The basic physical requirements described here are representative of those that are required daily as a Day Habilitation Direct Support Professional (DSP) to successfully perform the essential functions of this job.

  • Lifting/transferring approximately 50 to 100 pounds (usually with assistance).
  • Pushing/pulling (i.e. wheelchair) 100 pounds.
  • Frequent standing; walking; sitting; squatting; bending; reaching; fine motor skills.
