Essential Duties and Functions:

  • 1. Serve as a mentor and trainer for newly hired employees.
  • 2. Assist in supervising and transporting supported persons to community activities
  • 3. Implement all applicable policies, procedures, and/or legal commitments.
  • 4. Actively participate in staff meetings, trainings and special meetings.
  • 5. Be an advocate for all supported persons.
  • 6. Ensure all documentation is accurate and completed.
  • 7. Ensure supported persons receive medications/treatments as prescribed.
  • 8. Know and follow emergency home procedures for fire, tornadoes, accidents, and other serious incidents affecting supported persons or staff.
  • 9. Assist individual with daily hygiene needs (including personal care, brief change) if needed and daily cleaning of personal space (if needed).
  • 10. Assist with community activities such as purchasing clothing, personal items, and visits with friends and family.
  • 11. Implement the Individual Plan of Service/Person Centered Plan and strategies as written and at the frequency stated in the plan. Collect data accurately. Consistently follow written plan instructions and intervention strategies as instructed.
  • 12. Maintain cleanliness of the home through daily, weekly, and monthly cleaning duties. This includes but is not limited to laundry, dishes, bed making, and vacuuming while offering opportunities for individuals to participate, learn and increase independence.
  • 13. Maintain all required certifications such as CPR/First Aid, medication administration, etc.
  • 14. To facilitate teamwork between all team members via communication, sharing of responsibilities and a positive, constructive attitude.
  • 15. Completes other job duties as directed by supervisor.
