Job summary

Serenity Hospice Care in New Jersey is seeking a dedicated Hospice RN Team Lead to guide our Central team of nurses, develop patient care plans, and ensure care delivery aligns with established protocols and best practices.

You will supervise the interdisciplinary team, coordinate care, participate in on-call coverage, and travel as needed up to 50%. We offer a competitive salary and a comprehensive benefits package.

Qualifications

  • Current NJ RN license in good standing.
  • Minimum 2 years hospice experience.
  • Prior progressive management responsibilities.
  • Valid driver’s license with auto insurance and reliable transportation.
  • Proficiency with MS Word and Excel.
  • Excellent communication skills, written and verbal.
  • Ability to multitask and manage schedules.

Responsibilities

  • Coordinate and plan delivery of quality hospice care.
  • Evaluate services provided by the interdisciplinary team.
  • Monitor hospice licensure, regulatory and accreditation requirements.
  • Oversee care delivery, orientation, scheduling, education, and support of caregivers.
  • Be on-call as needed and support on-call staff.
  • Schedule and lead interdisciplinary team (IDT) meetings.
  • Develop physician relationships to familiarize them with hospice care.
  • Act as liaison between IDT, patients, and families.
  • Address concerns to support patient and customer relationships.
  • Facilitate interdepartmental communications.
  • Perform other duties as needed.

Job description

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice is looking to add a dedicated and organized Hospice RN Team Lead to our Central team. The Hospice RN Team Lead is responsible for providing leadership and direction to the designated team of nurses to ensure the highest standards of patient care.

The Team Lead is responsible for developing and implementing patient care plans, assessing patient needs, and ensuring that all care is delivered in accordance with established protocols and best practices.

Responsibilities:
  • Participates in the coordination and planning of the delivery of quality, cost-effective hospice care.
  • Evaluates patient services provided by the interdisciplinary team.
  • Maintains working knowledge of hospice state licensure, federal and accreditation requirements.
  • Monitor services and documentation to meet hospice policies and procedures, state, federal, and accreditation guidelines.
  • Serve as a liaison to the Administrator regarding clinical challenges and progress in patient care.
  • Supervise and participate in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team, including any contracted staff.
  • Be available on-call if needed. Provide support for on-call staff.
  • Schedule and run meetings for the interdisciplinary team (IDT).
  • Develop physician relationships to familiarize them with hospice care.
  • Act as a liaison between IDT members, patients, and families.
  • Contribute to effective patient and customer relationships by assisting patients, families, physicians, and staff to resolve expressed concerns.
  • Facilitate interdepartmental communications with co-workers and staff.
  • Perform other duties as needed.
Requirements:
  • Current NJ RN License in good standing
  • 2 Years of Hospice experience required
  • Prior progressive management responsibilities
  • Strong organizational skills.
  • Valid Drivers License with Auto Insurance.
  • Reliable transportation and willing to travel.
  • Proficient in Microsoft Word and Excel.
  • Strong computer skills and ability to learn new software.
  • Excellent communication skills, written and verbal.
  • Ability to multitask projects and assignments.
