You will provide direct supervision, personal care, and training in life skills and community integration, working one-on-one or in small groups. You will implement Individual Support Plans, monitor progress, coordinate with guardians and staff, and support daily activities while ensuring safety and dignity.

Qualifications

  • Must be 18 years of age or older.
  • At least 6 months of work experience in a human services field and/or with individuals with disabilities.
  • Ability to lift and move up to 50 lbs.
  • Good oral and written communication skills.

Responsibilities

  • Understand and implement ISPs including health, safety, recreation, and life skills.
  • Assess and evaluate progress toward ISP goals.
  • Identify community resources and facilitate participation in activities.
  • Provide supervision and training in life skills and social skills.
  • Attend to daily personal care with respect and dignity; assist with lifting as needed.
  • Apply strategies in Behavior Support Plans as applicable.
  • Assist in scheduling and transportation to community activities.
  • Monitor home and community safety and provide emergency training.
  • Maintain communication with guardians, families, and team members.
  • Complete monthly summaries for clients.

Description

SUMMARY:

Create opportunities for individuals with disabilities to live meaningful lives by providing support for individuals to participate in and fully benefit from services provided. Working one-on-one or within a small group setting, provide direct supervision and instruction to individuals, assist with daily personal care needs, and provide training and support for individuals to achieve outcomes in the areas of life skills, personal development, community integration, job skills and independent living. Also provides administrative support and coordination for assigned clients.

Essential Job Responsibilities:

  • Understand and implement Individual Support Plans of individuals served. ISP’s may include, but are not limited to, teaching, assisting, facilitating and building on the individual’s strengths in the following areas: health and safety, recreation, social skills, self-advocacy, employment, personal hygiene, meal planning and food preparation, shopping, budgeting and bill payment, and housekeeping and laundry.
  • Assess, implement and evaluate progress on identified goals/outcomes for individuals assigned, in accordance with the Individual Support Plan.
  • Identify community resources which could enhance an individual’s quality of life, and assist individuals in being active members of their community through participation in activities of their choice.
  • Provide direct, interactive supervision, instruction and training in a variety of life skill areas to engage with individuals served, such as: community networking and relationship building, utilizing community transportation services, and health and safety.
  • Attend to the daily personal, self-care needs (i.e. feeding, toileting, etc.) of individuals served in a timely manner and in a way that signifies respect and dignity to the individual. Requires lifting and transferring of individuals as necessary
  • Understand and implement the strategies and techniques in Behavior Support Plans, as applicable.
  • Administer medication as prescribed by the individual’s health care provider and in compliance with medication administration standards, and document such information in accordance with the individual’s support plan, physician’s orders and agency procedures.
  • Assist supervisor in designing and implementing an effective daily schedule that meets identified service standards and includes activities that are meaningful to individuals served
  • Provide transportation to and from community activities for individuals served using personal and/or agency vehicles.
  • Monitor the home and community environments for general cleanliness and potential safety hazards, provide training to individuals for emergencies, and ensure general safety precautions are in place
  • Promote, facilitate and establish linkages and community supports for individuals served
  • Schedule staff in a manner that provides needed coverage for clients and minimizes overtime pay, while ensuring that ISP outcomes can be adequately achieved.
  • Review staff time records and other documentation per agency timelines as requested, and reviewing for quality and accuracy.
  • Actively participate in client quarterly meetings, team meetings, and all other meetings as requested, taking action on all assigned items.
  • Complete monthly summaries for clients as directed.
  • Maintain regular communication with guardians, families, case managers and other team members.

Requirements

  • Must have and/or develop working knowledge and proficiency in Person Centered Planning and Positive Behavior Supports.
  • Ability to learn and effectively utilize identified computer software applications to accomplish tasks.
  • Oral/written communication skills to clearly and effectively express ideas.
  • Ability to positively participate as an effective team member.

Education and Experience:

  • Must be 18 years of age or older.
  • Must have at minimum 6 months of work experience in a human services field and/or experience with individuals with disabilities.

Physical Requirements:

  • Prolonged periods sitting at a desk and working on a computer
  • Must have ability to lift/move materials weighing up to 50 lbs. periodically
