Overview

Nurses in Acute Care Medicine manage patients with a full range of clinical issues, including blood dyscrasias, endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma, and other conditions. Due to patient acuity, we provide advanced training to bring new nurses to the highest level of functioning. Each unit is led by a clinical manager and charge nurse, supported by an advanced practice nurse, case managers, social workers, physical therapists, dietitians, and pharmacists. This multidisciplinary team collaborates with physicians, nurses, and patient care technicians to ensure quality patient care.

Preferred Qualifications

Role Purpose

Under the direction of a manager, plans, directs, and coordinates activities of a unit on a designated shift. Performs direct patient care as needed and functions as a clinical resource to nursing and support staff. Assesses, plans, evaluates, and documents the nursing process for the assigned unit as needed.

Responsibilities
  1. Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting. Critically assess patient data to proactively mitigate impending changes in condition and establish consults with the interprofessional team as appropriate.
  2. Leads the development and implementation of complex individual care plans with defined goals, in collaboration with the interprofessional team, patient, family, or caregiver, following established standards of nursing care. Plans reflect expert holistic clinical knowledge, anticipate barriers, incorporate team and family input, and aim to improve patient outcomes.
  3. Actively utilizes and promotes scientific inquiry for evidence–based practices, such as participating in journal clubs, facilitating practice changes, recommending new standards, conducting literature searches, and disseminating new knowledge. Supports nursing practice advancement through community service, patient education, advisory boards, or leadership roles.
  4. Analyzes practice standards gaps and provides feedback to peers in collaboration with leadership to enhance safe patient care. Evaluates the patient and family experience and adjusts care to improve satisfaction and quality.
  5. This is a safety–sensitive position; the ability to work alertly and safely is essential.
