Job summary

Centre for Neuro Skills in Emeryville, CA seeks a Clinical Case Manager to oversee clinical aspects of care across clinic and residential sites. The role involves developing and managing case plans, coordinating with families and external providers, and ensuring high-quality rehabilitation services.

Typical hours are Monday–Friday, 7:30am–4:30pm, with on-call and overtime. Minimum 3–5 years in rehabilitation required; CCM preferred and required upon eligibility.

Qualifications

• Successful candidates will have demonstrated passion for care and the ability to lead staff.
• Experience leading teams and evaluating staff performance is desirable.
• CCM certification or eligibility is preferred and CCM must be completed upon meeting eligibility guidelines.

Responsibilities

• Collects information about the patient’s situation to identify needs and develop a comprehensive case management plan.
• Plans objectives, goals and actions to meet patient needs and communicates issues with staff.
• Communicates with external stakeholders and families to coordinate services.
• Implements case management activities to achieve plan goals.
• Evaluates plan effectiveness and updates as needed.
