New Day Ranch MT is seeking a qualified case manager to deliver targeted services to youth deemed seriously emotionally disturbed. You will coordinate community supports, maintain ongoing contact with youth and families, and facilitate linkage to needed services.
The role emphasizes regular documentation, crisis planning, and discharge planning to promote continuity of care. Collaboration with treatment teams and advocacy for wrap-around services is required.
Provide targeted case management services for youth who are determined eligible as seriously emotionally disturbed per ARM 37.83.805. Facilitate the provision of community services appropriate and needed by the youth and family and ensure appropriate referrals for linkage services are available and used. Required to maintain regular contact with the youth, parent/guardian, family, and treatment team through in person visits or phone calls.

A minimum of a bachelor’s degree in human services field with at least one year of full-time experience serving people with mental illness. Individuals with other educational backgrounds, who as providers, consumers, or advocates of mental health services have developed the necessary skills may also qualify. Must be skilled in interviewing to gather data and completing needs assessments in preparation of narratives/report development of case management plans. Possess the basic knowledge of behavior management techniques, emotional and behavioral disorders, and individual and group communications. Have a basic understanding of Child Welfare Laws, including the Indian Child Welfare Act (ICWA) and be able to process for compliance.
$21.00 per hour
Monday - Friday
8:00 am - 4:00 pm
Plan for discharges in a way that affirms the progress and abilities of the participant and family to manage the treatment needs of the participant by establishing measurable discharge criteria.
Review discharge criteria monthly and assess the need for continuation of services.
